the District Registrar of Apprentices, a record in English showing in the case of each apprentice—

“ (a.) The name and age of the apprentice, and date of commencement of apprenticeship ;

“ (b.) The industry or branch thereof to which he is apprenticed ;

“ (c.) The hours of his employment on each day ;

“ (d.) The wages paid on each pay-day and the date thereof ;

“ (e.) Such other information as the District Registrar of Apprentices may require.

“ (2.) The Wages and Time Book in use for the time being and any such book used since the date of the commencement of the apprenticeship of the apprentice shall at all times be open to the inspection of the District Registrar of Apprentices.

“ (3.) The District Registrar of Apprentices may at any time require the employer to verify the entries in the Wages and Time Book in such manner as the District Registrar may think fit.”

(8.) The following forms Nos. 13 and 14 are hereby inserted after form No. 12 :—

“ Section 5, Apprentices Act, 1923.

“ APPLICATION FOR APPRENTICESHIP ORDER.

“ To the District Registrar of Apprentices.

“ TAKE notice that I [we] hereby make application to the Court of Arbitration for an apprenticeship order in the [Industry or branch thereof] in the [Locality].

(a) or (b)
I [We] attach four copies of No recommendation of an Apprenticeship Committee has been made.
the recommendations of the Apprenticeship Committee set up for the [Industry] in the [Locality].

[Strike out (a) or (b), as the case may be.]

“ The address of the Secretary of the Apprenticeship Committee is

“ Dated at this day of , 192 .

“ [Signature(s).]

“ [If the application is made on behalf of another person or of an Apprenticeship Committee, state accordingly]—On behalf of .”

“ [Form 14.

“ Section 5, Apprentices Act, 1923.

“ APPLICATION FOR AMENDMENT OF APPRENTICESHIP ORDER.

“ To the District Registrar of Apprentices.

“ TAKE notice that I [we] hereby make application to the Court of Arbitration for an amendment of the apprenticeship order dated the day of , 192 , as follows :—

“ [Set out particulars of the amendment desired.]

(a) or (b)
I [We] attach four copies of No recommendation of an Apprenticeship Committee has been made.
