STATUTORY DECLARATION.

(2.) That the above is a full and correct statement of the names, addresses, and occupations of the officers, trustees, and auditors of the association and of the number of unions affiliated with the association whose subscriptions were not more than twelve months in arrears as on the 31st day of December, 19 .

  1. Form No. 2C is hereby revoked.

  2. Form No. 5A is hereby amended (a) by inserting the words “by secret ballot” after the word “passed” in paragraph (7); and (b) by deleting the words “and confirmed by a subsequent ballot of the members” in paragraph (7).

  3. Form No. 5B is hereby revoked.

  4. Form No. 5C is hereby amended (a) by inserting the words “by secret ballot” after the word “passed” in paragraph 3; (b) by omitting paragraph 4; and (c) by omitting paragraph 5, and substituting the following paragraph:—

“5. That the result of the ballot was as follows: For the resolution, votes; against the resolution, votes.”

  1. Form No. 9A is hereby amended by omitting the words “has heard the dispute and that no settlement thereof has been arrived at,” and substituting therefor the words “is satisfied that a settlement of the dispute will not thereby be arrived at.”

  2. The following new form No. 9E is hereby inserted after form No. 9D :—

APPLICATION THAT INDUSTRIAL DISPUTE AFFECTING TWO OR MORE INDUSTRIAL DISTRICTS BE HEARD BY A COUNCIL OF CONCILIATION.

WHEREAS an industrial dispute relating to two or more industrial districts, that is to say, the industrial districts of , has arisen between [Name of applicant] and [Name of respondents], and it is expedient that the dispute should be heard by a Council of Conciliation, application is hereby made, in pursuance of section 2 of the Industrial Conciliation and Arbitration Amendment Act, 1922, that the said dispute should be so heard.

(1.) This application is made by [Name of applicant] (hereinafter referred to as “the applicant”).

(2.) It is desired that [Names of respondents] (hereinafter referred to as “the respondents”) be made parties to the proceedings in the matter of the said dispute.

(3.) The following is a general statement of the nature of the dispute [Describe dispute].

(4.) The applicant makes the following claims against the respondents in the matter of the said dispute [Set out a detailed statement of such claims].

(5.) The applicant recommends that one [or two, &c., up to six] assessor[s] sit with the Commissioner in the hearing and settlement of the said dispute.

(6.) The persons so recommended as assessors are [Names, occupations, and addresses of proposed assessors].

(7.) This application has been approved by the members of each of the unions concerned by a resolution passed by secret ballot at a special meeting of the union, a majority of the votes recorded being in favour thereof, and the results of such ballots have been recorded in the minutes of the said meetings. A certificate under the hand of the chairman of each of the said meetings that the said meeting was properly constituted and held, and setting forth the nature of the proposal submitted at that meeting and the result of the voting thereon, is hereto attached.

Form No. 10A is hereby amended (a) by omitting the words “To His Excellency the Governor,” and substituting therefor the words “To the Registrar of Industrial Unions, Wellington”; and (b) by inserting the words “in accordance with a resolution passed at a special meeting thereof” before the words “hereby recommends.”

  2. Form No. 12A is hereby amended by omitting the words “no settlement of the above-mentioned dispute has been arrived at by the Council of Conciliation appointed for the hearing of the same,” and substituting the words “the Council appointed for the hearing of the above-mentioned dispute is satisfied that a settlement of the dispute will not thereby be arrived at.”

  3. Form No. 12C is hereby amended by omitting the words “no settlement of the above-mentioned dispute has been arrived at by the Council of Conciliation appointed to hear the same,” and substituting the words “the Council of Conciliation appointed for the hearing of the above-mentioned dispute is satisfied that a settlement of the dispute will not thereby be arrived at.”

  4. Form No. 13A is hereby amended (a) by inserting the words “by secret ballot” after the word “passed” in para.
