Approval of Taranaki District Health Board Alcohol and Drug Service and Clinician to Provide Substitution Treatment Under Section 24 of the Misuse of Drugs Act 1975
Pursuant to section 24(7)(b) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, specify Taranaki District Health Board Alcohol and Drug Service as a service that may provide treatment with controlled drugs for people who are dependent on controlled drugs, at the following place:
Taranaki Base Hospital
David Street
New Plymouth 4310
And, pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, acting under delegated authority from the Minister of Health, I specify
Dr Sharat Shetty
as a medical practitioner, may:
a. prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for Taranaki District Health Board Alcohol and Drug Service;
b. authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:
i. prescribe, administer or supply controlled drugs for the treatment of dependence;
ii. authorise in writing general practitioners receiving clients from Taranaki District Health Board Alcohol and Drug Service as specified under subsection (2)(d); and
c. authorise in writing general practitioners receiving clients from Taranaki District Health Board Alcohol and Drug Service as specified under subsection (2)(d).
Conditions

Controlled drugs may only be prescribed, administered or supplied to people who are or have been clients of Taranaki District Health Board Alcohol and Drug Service.

This approval expires on 31 December 2017.

Appointment of Director of Area Mental Health Services
Pursuant to section 92 of the Mental Health (Compulsory Assessment and Treatment) Act 1992, I, Chai Chuah, Director-General of Health, issue the following notice, effective from 12 December 2016.

This notice shall be read together with Appointment of Directors of Area Mental Health Services (“principal notice”) (New Zealand Gazette, 17 October 2013, No. 143, page 3909).

Schedule 1 to the principal notice is amended in respect of "Tairawhiti DHB":
a. by omitting the name and title"Michael Norfolk, registered nurse, of Gisborne"
b. and by adding the name and title
"Susan Mackersey, psychiatrist, of Tauranga"

This amendment to the principal notice shall expire on 31 January 2017.
