Pursuant to section 52(1) of the New Zealand Public Health and Disability Act 2000, and sections 28(1)(a) and 32(2) and clause 1(1) of Schedule 5 to the Crown Entities Act 2004, and notice given under section 28(2) of the Crown Entities Act 2004, I have reappointed

as a member and Chair of the board of the Pharmaceutical Management Agency for a term of office commencing on 27 October 2015 and ending on 31 July 2018; and reappointed
Janis White
as a member of the board of the Pharmaceutical Management Agency for a term of office commencing on 27 October 2015 and ending on 31 July 2018.

Cancellation of Licence for an Early Childhood Service
Pursuant to Regulation 32(1)(d)(i) of the Education (Early Childhood Services) Regulations 2008, and acting under authority delegated by the Secretary for Education, I hereby cancel the licence dated 10 May 2010, which was granted under those Regulations to Sue Dobbs in respect of Crackerjacks Preschool Limited Seniors (30286), situated at 192 Domain Road, Te Awamutu.
This notice shall take effect the day after the date of its notification in the New Zealand Gazette.
