Approval of Waikato District Health Board Community Alcohol and Drug Service and Clinician to Provide Substitution Treatment Under Section 24 of the Misuse of Drugs Act 1975

Pursuant to section 24(7)(b) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, specify Waikato District Health Board Community Alcohol and Drug Service (Waikato CADS) as a service that may provide treatment with controlled drugs, for people who are dependent on controlled drugs, at the following places:

Community Alcohol & Drug Service
193 London Street
Hamilton 3204

Thames Hospital
610 Mackay Street
Thames 3500

242 Bank Street
Te Awamutu 3800

And, pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, acting under delegated authority from the Minister of Health, I specify

Dr Andrew Vincent Darby

as a medical practitioner, may:

a. prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for Waikato CADS;

b. authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:

i. prescribe, administer or supply controlled drugs for the treatment of dependence;

ii. authorise in writing general practitioners receiving clients from Waikato CADS as specified under subsection (2)(d); and

c. authorise in writing general practitioners receiving clients from Waikato CADS as specified under subsection (2)(d).

Conditions

  1. Controlled drugs may only be prescribed, administered or supplied to people who are or have been clients of Waikato CADS.

  2. This approval takes effect on 14 September 2015.

  3. This approval expires on 31 December 2017.

Revocation of Notices Under Section 24 of the Misuse of Drugs Act 1975

Pursuant to section 24(7) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, revoke the approval of the Waikato District Health Board Alcohol and Drug service and medical practitioner issued under this section on 3 July 2014 (New Zealand Gazette, 3 July 2014, No. 70, page 51).
