Approval of Rata Alcohol and Other Drugs Service and Clinician to Provide Substitution Treatment Under Section 24 of the Misuse of Drugs Act 1975

Pursuant to section 24(7)(b) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, specify the Rata Alcohol and Other Drugs Service as a service that may provide treatment with controlled drugs for people who are dependent on controlled drugs, at the following places:

Grey Base Hospital
High Street
Greymouth 780; and

Buller Mental Health Service
Derby Street
Westport 7825; and

Hokitika Health Centre
Sewell Street
Hokitika 7810.

And, pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, acting under delegated authority from the Minister of Health, I specify

Dr Alfred Valare Dell’Ario
as a medical practitioner, may:

(a) prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for Rata Alcohol and Other Drugs Service;

(b) authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:

(i) prescribe, administer or supply controlled drugs for the treatment of dependence;

(ii) to authorise in writing general practitioners receiving clients from Rata Alcohol and Other Drugs Service as specified under subsection (2)(d); and

(c) authorise in writing general practitioners receiving clients from Rata Alcohol and Other Drugs Service as specified under subsection (2)(d).

Conditions

  1. Controlled drugs may only be prescribed, administered or supplied to people who are or have been clients of Rata Alcohol and Other Drugs Service.

  2. This approval expires on 31 December 2017.

Approval of South Canterbury District Health Board Methadone Maintenance Treatment Programme and Clinician to Provide Substitution Treatment Under Section 24 of the Misuse of Drugs Act 1975

Pursuant to section 24(7)(b) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, specify the South Canterbury District Health Board Methadone Maintenance Treatment Programme as a service that may provide treatment with controlled drugs for people who are dependent on controlled drugs, at the following place:

Timaru Hospital
Kensington Centre
Queen Street
Timaru 7910.

And, pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, acting under delegated authority from the Minister of Health, I specify

Dr Deirdre Mary Love
as a medical practitioner, may:

(a) prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for South Canterbury District Health Board Methadone Maintenance Treatment Programme;

(b) authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:

(i) prescribe, administer or supply controlled drugs for the treatment of dependence;

(ii) to authorise in writing general practitioners receiving clients from South Canterbury District Health Board Methadone Maintenance Treatment Programme as specified under subsection (2)(d); and

(c) authorise in writing general practitioners receiving clients from South Canterbury District Health Board Methadone Maintenance Treatment Programme as specified under subsection (2)(d).

Conditions

  1. Controlled drugs may only be prescribed, administered or supplied to people who are or have been clients of South Canterbury District Health Board Methadone Maintenance Treatment Programme.

  2. This approval expires on 31 December 2017.

Approval of Otago Community Alcohol and Drug Service and Clinician to Provide Substitution Treatment Under Section 24 of the Misuse of Drugs Act 1975

Pursuant to section 24(7)(b) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, specify the Otago Community Alcohol and Drug Service (“Otago CADS”) as a service that may provide treatment with controlled drugs for people who are dependent on controlled drugs, at the following places:

Waitaki Community Mental Health
2 Trent Street
Oamaru 9400; and

Clutha Mental Health
24 Clyde Street

And, pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, acting under delegated authority from the Minister of Health, I specify

Dr Gavin Stuart Cape
as a medical practitioner, may:

(a) prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for Otago CADS;

(b) authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:
