Approval of Tairawhiti District Health Community Mental Health and Addiction Service and Clinician to Provide Substitution Treatment Under Section 24 of the Misuse of Drugs Act 1975
Pursuant to section 24(7)(b) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, specify the Tairawhiti District Health Community Mental Health and Addiction Service as a service that may provide treatment with controlled drugs for people who are dependent on controlled drugs, at the following place:
Community Mental Health and Addiction Service
110 Peel Street
Gisborne 4010.
And, pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, acting under delegated authority from the Minister of Health, I specify
Dr Patrick James McHugh
as a medical practitioner, may:
(a) prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for Tairawhiti District Health Community Mental Health and Addiction Service;
(b) authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:
(i) prescribe, administer or supply controlled drugs for the treatment of dependence;
(ii) to authorise in writing general practitioners receiving clients from Tairawhiti District Health Community Mental Health and Addiction Service as specified under subsection (2)(d); and
(c) authorise in writing general practitioners receiving clients from Tairawhiti District Health Community Mental Health and Addiction Service as specified under subsection (2)(d).
Conditions
-
Controlled drugs may only be prescribed, administered, or supplied to people who are or have been clients of Tairawhiti District Health Community Mental Health and Addiction Service.

This approval expires on 31 December 2017.

Approval of Lakes Opioid Treatment Service and Clinician to Provide Substitution Treatment Under Section 24 of the Misuse of Drugs Act 1975
Pursuant to section 24(7)(b) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, specify the Lakes Opioid Treatment Service as a service that may provide treatment with controlled drugs for people who are dependent on controlled drugs, at the following places:
Te Ngako Community Mental Health Services
1144 Pukaki Street
Rotorua 3010; and
Ruia Te Hua Oranga
3 Gascoigne Street
Taupo 3330
And, pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, acting under delegated authority from the Minister of Health, I specify
Dr Adrian John Gray
as a medical practitioner, may:
(a) prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for Lakes Opioid Treatment Service;
(b) authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:
(i) prescribe, administer or supply controlled drugs for the treatment of dependence;
(ii) to authorise in writing general practitioners receiving clients from Lakes Opioid Treatment Service as specified under subsection (2)(d); and
(c) authorise in writing general practitioners receiving clients from Lakes Opioid Treatment Service as specified under subsection (2)(d).
Conditions

Controlled drugs may only be prescribed, administered, or supplied to people who are or have been clients of Lakes Opioid Treatment Service.

And, pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, acting under delegated authority from the Minister of Health, I specify

as a medical practitioner, may:
(a) prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for Taranaki District Health Board Alcohol and Drug Service;
(b) authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:
(i) prescribe, administer or supply controlled drugs for the treatment of dependence;
(ii) to authorise in writing general practitioners receiving clients from Taranaki District Health Board Alcohol and Drug Service as specified under subsection (2)(d); and
