as a medical practitioner, may:

(a) prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for CADS AOTS;

(b) authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:

(i) prescribe, administer or supply controlled drugs for the treatment of dependence;

(ii) to authorise in writing general practitioners receiving clients from CADS AOTS as specified under subsection (2)(d); and

(c) authorise in writing general practitioners receiving clients from CADS AOTS as specified under subsection (2)(d).

Conditions

  1. Controlled drugs may only be prescribed, administered or supplied to people who are or have been clients of CADS AOTS.

  2. This approval expires on 31 December 2017.

Approval of Waikato District Health Board Community Alcohol and Drug Service and Clinician to Provide Substitution Treatment Under Section 24 of the Misuse of Drugs Act 1975

Pursuant to section 24(7)(b) of the Misuse of Drugs Act 1975, I, John Crawshaw, acting under delegated authority from the Minister of Health, specify Waikato District Health Board Community Alcohol and Drug Service (β€œWaikato CADS”) as a service that may provide treatment with controlled drugs for people who are dependent on controlled drugs, at the following places:

Te Awamutu 3800; and

Community Alcohol & Drug Service
193 London Street
Hamilton 3204; and

Thames Hospital
610 Mackay Street
Thames 3500;

And, pursuant to section 24(7)(a) of the Misuse of Drugs Act 1975, acting under delegated authority from the Minister of Health, I specify

as a medical practitioner, may:

(a) prescribe, administer or supply controlled drugs for the treatment of dependence to controlled drugs as the lead clinician for Waikato CADS;

(i) prescribe, administer or supply controlled drugs for the treatment of dependence;

(b) authorise in writing service/clinic medical practitioners as specified under subsection (2)(b) to:

(i) prescribe, administer or supply controlled drugs for the treatment of dependence;

(ii) to authorise in writing general practitioners receiving clients from Bay of Plenty District Health Board Addiction Service as specified under subsection (2)(d); and

(c) authorise in writing general practitioners receiving clients from Bay of Plenty District Health Board Addiction Service as specified under subsection (2)(d).

Conditions

  1. Controlled drugs may only be prescribed, administered or supplied to people who are or have been clients of Bay of Plenty District Health Board Addiction Service.

  2. This approval expires on 31 December 2015.
