• The category of gasfitting.
  Note: Available categories are: campervan, caravan, commercial, domestic, industrial, other and temporary.
• The type of gasfitting, as defined in Regulation 44(1) of the Gas (Safety and Measurement) Regulations 2010.
• The gas type.
• A description of the gasfitting work carried out.
  Note: This may include a reference to the work number or detailed specification of the work that is held by the owner or operator of the installation.
• The working pressure of the installation or that part of the system worked on.
• Confirmation that the required testing and commissioning has been carried out, the result of testing and commissioning is satisfactory and has been recorded.

The information must be completed in the order specified and be completed on the Plumbers Gasfitters and Drainlayers Board’s online electronic certification database.
Note: Other information that may appear on the form when implemented is not mandatory for gasfitting undertaken under section 22 of the PG&D Act, but may be completed at the discretion of the certifier.

Gasfitting carried out under section 62 of the PG&D Act (Employer licence)

The form of certificate for certification of gasfitting carried out under section 62 of the PG&D Act must contain the following information:

• A unique certificate number.
• Date of certification.
• Identification of the installation.
• Physical address of the installation.
  Note: This is the street address, not a postal address.
  Note: Where the installation is in or on a vehicle or a vessel, that vehicle or vessel shall be unambiguously identified.
• Identity of the licensee.
• Identity of the certifier.
• The category of gasfitting.
  Note: Available categories are: campervan, caravan, commercial, domestic, industrial, other and temporary.
• The type of gasfitting, as defined in Regulation 44(1) of the Gas (Safety and Measurement) Regulations 2010.
• The gas type.
• The working pressure of that part of the system worked on.
