Order of the Nursing Council in Disciplinary Session

in Respect of a Registered Psychiatric Nurse

Pursuant to sections 42 (1) (b), 42 (2) (a), 42 (3) and 48A of
the Nurses Act 1977, the Nursing Council of New Zealand
(“the council”), in a decision dated 4 May 2004, ordered that
the name Grant Allan Amer, registered psychiatric nurse,
be removed from the Register of Psychiatric Nurses and that
he may reapply for reinstatement to the Register after a period
of two years.

The council also ordered that Grant Allan Amer pay
$13,430.00 (being 30%) of the actual costs and expenses of
and incidental to the investigation by the preliminary
proceedings committee and inquiry by the council, and that
a notice stating the effect of the orders be published in the
New Zealand Gazette, Kai Tiaki: Nursing New Zealand and
released to accredited members of the media with the
identifying details of other health professionals, institutions
and clients suppressed.

Nurses (Fees) Notice 2004

Pursuant to section 14A of the Nurses Act 1977, the following
notice is given.

Notice

1. Title and commencement—This notice may be cited
   as the Nurses (Fees) Notice 2004 and shall come into
   force on 1 July 2004.

2. Fees—The Nursing Council of New Zealand (“the
   council”) sets the following fee payable to the council
   specified in the Schedule to this notice.

3. Tax—The fees are inclusive of goods and services tax.

4. Notice—This notice prescribes the fee payable under
   section 24 of the Nurses Act 1977 and pursuant to section
   14A (1) (b) of the Nurses Act 1977.

Fee Payable to the Nursing Council of New Zealand

Pursuant to section 14A (1) (b) of the Nurses Act 1977, the
council gives notice that the following fee is payable
(includes G.S.T.), effective from 1 July 2004.

Nurse Practitioner Endorsement Application
Fee (section 24 Nurses Act 1977) $400.00¹

Key to Annotation

¹ The fee payable is for nurse practitioner endorsement on
the council’s Register, namely: an addition to the Register
under section 24 of the Nurses Act 1977.

Optometrists and Dispensing Opticians (Fees)

Amendment Notice 2004

Pursuant to section 13A (1) (a) of the Optometrists and
Dispensing Opticians Act 1976, the following notice is given.

Notice

1. Title and commencement—(1) This notice may be
   cited as the Optometrists and Dispensing Opticians
   (Fees) Amendment Notice 2004 and should be read in
   conjunction with the notice published in the New Zealand
   Gazette, 4 December 2003, No. 166, page 4628.

(2) This notice comes into force on 19 July 2004.

2. Fee—The Opticians Board sets the following fee
   payable to the board specified in the attached Schedule.

3. Tax—The fee is inclusive of goods and services tax.

Application for registration from an overseas
trained optometrist or dispensing optician
under section 18 (1) of the Optometrists
and Dispensing Opticians Act 1976. This
excludes applications from optometrists and
dispensing opticians with an Australian
qualification, and applications made under
the Trans Tasman Mutual Recognition Act
1997. 550.00
