The Alcoholic Liquor Advisory Council Levy Notice 1995
Pursuant to section 27 (1) of the Alcoholic Liquor Advisory Council Act 1976, the Minister of Health hereby gives the following notice:

This notice may be cited as the Alcoholic Liquor Advisory Council Levy Notice 1995.

The rate of levy payable under section 28 of the Alcoholic Liquor Advisory Council Act 1976, for the year ended 30 June 1996, shall be as follows:
Beer 0.84 c/litre plus G.S.T.
Spirits 27.60 c/litre of alcohol (LAL) plus G.S.T.
Fortified wine 5.06 c/litre plus G.S.T.
Unfortified wine 3.10 c/litre plus G.S.T.
Spirits less than 23% alcohol by volume 5.06 c/litre plus G.S.T.

Health and Disability Services Act 1993
Appointments to the Public Health Commission
Pursuant to section 30 (2) of the Health and Disability Services Act 1993, the following persons are hereby appointed as directors of the Public Health Commission for terms commencing on the 1st day of July 1995 and expiring on the 31st day of December:

Medicines Act 1981
Revocation or Prohibition on the Prescribing of Medicines
Pursuant to section 48 (3) of the Medicines Act 1981, I, Maurice Donald Williamson, Associate Minister of Health, on the recommendation of the Medical Council of New Zealand, hereby revoke the notice, referred to in the Schedule hereto, prohibiting Raymond Reginald Harvey, medical practitioner of Tauranga, from prescribing benzodiazepine and its derivatives.

Revocation of Prohibition on the Prescribing of Controlled Drugs
Pursuant to section 23 (3) of the Misuse of Drugs Act 1975, I, Maurice Donald Williamson, Associate Minister of Health, on the recommendation of the Medical Council of New Zealand, hereby revoke the notice, referred to in the Schedule hereto, prohibiting Raymond Reginald Harvey, medical practitioner of Tauranga, from prescribing controlled drugs.

Her Excellency the Governor-General in Council
Pursuant to section 22 (1) of the Citizens Initiated Referenda Act 1993, Her Excellency the Governor-General, acting by and with the advice and consent of the Executive Council, and following the presentation to the House of Representatives on the 30th day of May 1995 of an indicative referendum petition which had been promoted by the New Zealand Professional Firefighters Union and which had been certified correct under section 18 (1) (a) of that Act, hereby makes the following order.
Order
- Title and commencement—(1) This order may be cited as the Citizens Initiated Referendum (Appointed Day) Order 1995.
(2) This order shall come into force on the day after the date of its publication in the Gazette.
- Appointed day—The 2nd day of December 1995 is hereby appointed as the day on which an indicative referendum shall be held under the Citizens Initiated Referenda Act 1993 on the following question, namely,—
Should the number of professional firefighters employed full-time in the New Zealand Fire Service be reduced below the number employed on 1 January 1995?
