SCHEDULE
CONDITIONS
(1) This licence is subject to the Foreshore Licence Regulations 1960, and the provisions of those regulations shall, so far as applicable, apply hereto.
(2) The term of the licence shall be one year from the 1st day of February 1977.
(3) The premium payable by the licensee shall be fifteen dollars ($15) and the annual sum so payable by the licensee shall be one hundred and fifty dollars ($150); provided always that the Minister may review the annual sum payable at the end of the first year or any subsequent year of the said term.

Licensing the Waipuna Boating Club (Incorporated) to Occupy Sites for Pile Moorings in the Tamaki River
PURSUANT to section 162 of the Harbours Act 1950, I, Owen John Conway, of the Ministry of Transport, in exercise of powers delegated by the Minister of Transport, hereby license and permit the Waipuna Boating Club (Incorporated) (hereinafter called the licensee, which term shall include its successors or assigns unless the context requires a different construction) to use and occupy a part of the bed of the Tamaki River as shown on plans marked M.D. 15273, MD(N) 230, and MD(N) 356, and deposited in the office of the Ministry of Transport at Auckland, for the purpose of maintaining thereon pile moorings as shown on the said plans, such licence to be held and enjoyed by the licensee upon and subject to the terms and conditions set forth in the Schedule hereto. The licence of 15 February 1977*, authorising the Waipuna Boating Club (Incorporated) to occupy sites for pile moorings in the Tamaki River, is hereby revoked.
SCHEDULE

Poisons Regulations 1964—Approval of Alternative Form of Record
NOTICE is hereby given that pursuant to regulation 61 of the Poisons Regulations 1964*, the Registrar has approved a record in the form of an individual patient-card index, of the dispensing or supply on prescription of any poison as a medicine, such record being made and maintained in accordance with the system hereinafter described:

An individual card shall be allocated either to each patient or family, and be filed in alphabetical and numerical sequence in an appropriate metal filing cabinet.

Where a patient’s surname is repeated, the cards shall be filed alphabetically by way of each patient’s address.

This card system shall be supplemented by a separate day book, which will provide a cross reference, by recording serially by number, the patient’s surname to which that number applies.

A separate record is to be maintained, of the dispensing or supply on prescription of any Class B Controlled Drug, as required by regulations 37 or 39 of the Misuse of Drugs Regulations 1977†.

Otago Raspberry Marketing Committee Election (No. 1705 Ag. 12/3/17)
PURSUANT to the Second Schedule to the Raspberry Marketing Regulations, notice is hereby given that the roll of those persons qualified to vote for the election of two producers’ representatives in the northern ward to the Otago Raspberry Marketing Committee will be open for inspection during ordinary office hours at the following places, viz, Ministry of Agriculture and Fisheries, Head Office, Wellington, also at the offices of the Ministry of Agriculture and Fisheries at Dunedin, Timaru, and Waimate, the Post Office, Temuka, and at the office of A. P. Hayes (Secretary), 37A Thames Street, Oamaru.
The roll will be available for public inspection for a period of 7 days from 26 May 1977, during which period any person may lodge with the Returning Officer, an objection in writing under his hand to any entry on the roll.
Nomination forms may be obtained on application to any of the above offices or from the Returning Officer, Ministry of Agriculture and Fisheries, Dunedin. Nominations must be in the hands of the returning officer not later than noon on the 13th day of July 1977.
