Article 391: Cancel, and substitute:

391. Medical Report on an Officer or Rating prior to Discharge or Reversion to the Royal Navy.—Each officer and rating serving in the New Zealand Naval Forces is to be examined on the following occasions by the medical officer of his ship or establishment to determine whether he suffers from any disability either attributable to or aggravated by his service in the New Zealand Naval Forces:—

“ (i) On final discharge to shore whether he belongs to the New Zealand Naval Forces permanently or is on loan.

“ (ii) Before reversion to the Royal Navy on completion of his period of loan.

“ (iii) On completion of his Royal Navy engagement but continuing to serve under a loan agreement.

“ 2. The medical and dental examination is to be carried out as laid down in Confidential Admiralty Fleet Order 1990/24.

“ 3. Form N.Z.D. 50 is to be completed at this examination in accordance with the instructions on the form and forwarded to the Director of Medical Services for transmission to the Naval Secretary. The forms are to be forwarded to Navy Office in duplicate in respect of all officers and ratings who have served in the Royal Navy.

“ 4. Medical officers are to exercise the greatest care in completing the form and to ensure that all details are correctly furnished. The Naval Board should be in a position to know the exact nature and degree of any disability at the time of the examination, answers to questions 6 to 9 being framed accordingly.

“ 5. Form N.Z.D. 50 is not required when an officer or rating is invalided from the Naval Service.

“ 6. When personnel are found to be suffering from any disability it will be necessary to obtain the services of a second medical officer. When no other ship is in company at ports on the New Zealand Station the local surgeon and agent should be employed. When a ship is paying off in England application should be made for the services of a medical officer to be loaned from the Naval Barracks at the port where the ship is lying.

“ 7. Medical and Dental History Sheets.—Medical and dental history sheets are to be completed to show the results of the above examinations. Where the ratings continue to serve in the New Zealand Naval Forces the history sheets are to continue in use, but on discharge to shore in New Zealand or reversion to the Royal Navy they are to be forwarded to Navy Office through the Director of Medical Services. History sheets in respect of personnel who have completed their Royal Navy engagement but who continue to serve in the New Zealand Naval Forces will be forwarded on their discharge to shore in England to the Medical Director-General, Admiralty, by Navy Office.”

Remarks.—Two separate Journals are to be rendered, one containing particulars of both New Zealand and Imperial personnel and the other particulars of Imperial personnel only. In the complete return the columns of forms M.179D and M.179E should be subdivided to show New Zealand and Imperial statistics separately.”
