Transport Licensing (Commercial Aircraft Services) Act, 1934.
APPLICATION FOR SPECIAL AIRCRAFT SERVICE LICENSE BY AERO CLUB.
To the Commissioner of Transport, Wellington, C.1 (forwarded through the New Zealand Aero Club, Incorporated).
I, THE undersigned, hereby apply for a Special Aircraft Service License under the above Act, to be issued to the club named below, to operate an aircraft service, particulars of which are given below, and I declare that the statements made herein are true, correct, and complete in every particular, and that I am duly authorized by the club to make this application.

Recommendation of the New Zealand Aero Club, Incorporated, as to minimum fares and charges may be endorsed on Schedule.] 5.
Official receipts must be issued for the receipt of all public moneys. If you do not receive within ten days an official receipt for the fee enclosed with this application, please communicate with the Commissioner of Transport, Wellington, C.1.

A “special” license is available for a four-years period from the date of the passing of the above-entitled Act to aero clubs incorporated at that date, and authorizes the use in connection with the service to which the license relates only of aircraft then owned by the club or then otherwise lawfully in its possession. For a service to be conducted by any other club or for any other period, or in connection with which it is desired to use any other aircraft, a continuous or temporary license should be applied for.
These types of license are described in the Act (section 10) as follows:—
(a) A continuous license—meaning thereby a license other than a temporary license.
(b) A temporary license—meaning thereby a license for a service to be carried on for a specified period of not more than seven days or a license for any specified special occasion or occasions.
Application for a special license should be made on Form T.L. 41 (green form).
Application for a continuous license should be made on Form T.L. 42 (pink form).
Application for a temporary license should be made on Form T.L. 43 (blue form).
Application for renewal of a continuous license should be made on Form T.L. 44 (pink form).

NOTE.—A bank receipt must accompany this application showing that a fee of £2 in respect of it has been paid into the Public Account at the Bank of New Zealand to the credit of the Consolidated Fund.
(Attention is directed to the fact that if any information supplied on this form is false or misleading it renders the applicant liable to a fine of £10.)

APPLICATION FOR CONTINUOUS AIRCRAFT-SERVICE LICENSE.
To the Commissioner of Transport, Wellington, C.1.
I, THE undersigned, hereby apply for a Continuous Aircraft-service License under the above Act, to operate an aircraft service, particulars of which are given below, and I declare that to the best of my knowledge and belief the statements made herein are true, correct, and complete in every particular.
I. Full name of applicant [in block letters: in case of a partnership, give full name of every partner, surnames last: in other cases of individuals, surname last.]
The application of a registered company or other corporate body must be under seal or signed by the secretary or manager or other responsible official in each case duly authorized.
II. Trade name [Here give firm name of a partnership: trade-name, if any, of a service not carried on solely under the name of applicant.]
The application of a partnership firm must be signed by one of the partners.
III. Business address [In case of a registered company, &c., state registered office.]
IV. If application is not made by applicant in person,—
