Form 1.—Application for License.

[Full name and address of applicant] hereby makes application for a license, under the provisions of “The Cyanide Process Gold-extraction Act, 1897,” for the use and employment of the therein-mentioned patent rights in the Mine [or Battery], situate at , for the extraction of gold and silver from quartz or other ores.

Form 2.—License.
Pursuant to the provisions of “The Cyanide Process Gold-extraction Act, 1897,” and the regulations thereunder, and to the application in this behalf dated the day of , 18 , [Full name and address of licensee] is hereby licensed to use and employ in the applicant’s mine situate in [Mining district, if any, and locality], and known as [Name of mine], the patent rights referred to in the said Act; subject nevertheless to the payment of the royalty and the observance of the terms and conditions prescribed by the said Act and the regulations from time to time in force thereunder.

Form 3.—Return to be furnished to Receiver of Gold Revenue by Licensees.

- That the following statement contains a full, true, and accurate return of all quartz or other ores treated in the said mine during the period [or respective periods] therein mentioned whilst using or employing the patent rights referred to in the said Act, the number of ounces of gold and silver respectively produced thereby, the values thereof, and the amount of royalty payable in respect thereof, as prescribed by the regulations under the said Act:—
