I, LOUIS DAVIES, hereby make application to register the Golden Bell Quartzmining Company, Registered, under the provisions of the Mining Companies Limited Liability Act, 1865, and the Mining Companies Limited Liability Acts Amendment Acts, 1869, and 1871; and I do solemnly and sincerely declare that the following statement is to the best of my belief and knowledge true in every particular, namely:—

The name and style of the Company is the Golden Bell Quartzmining Company, Registered.

The place of operations is at Frank McLean's and party's claim, west of Adam Smith's line of reef, Inangahua, in the Province of Nelson.

The nominal capital of the Company is twelve thousand pounds in twelve thousand shares of one pound each.

The amount already paid up in cash is five hundred pounds and the amount allotted to shareholders in partly paid-up shares is five thousand five hundred pounds.

The name of the manager is Louis Davies.

The Office of the Company is at Broadway, Reefton, in the Province of Nelson.
