I, THE undersigned, WALTER SULLY, hereby make application to register the "COLLARBONE GOLD MINING COMPANY," under the provisions of "The Mining Companies Limited Liability Act, 1865," and "The Mining Companies Limited Liability Act Amendment Act, 1869," and I do solemnly and sincerely declare that the following statement is, to the best of my belief and knowledge, true in every particular, viz.:—

1. The name and style of the Company is the "Collarbone Gold Mining Company."

2. The place of operations is at Collarbone Karaka, at the Thames Gold Field, in the Province of Auckland, in the Colony of New Zealand.

3. The nominal capital of the Company is Twenty-nine thousand (£29,000) Pounds, in Twenty-nine thousand shares of One Pound each (£1).

4. The amount paid up in cash is nil; in partly paid up shares is Twenty-five thousand three hundred and seventy-five pounds, (£25,375) being the estimated value of the mine.

5. The name of the Manager is Walter Sully.

6. The Office of the Company is at Pollen-street, Shortland.
