Pursuant to section 23(4A) of the Medicines Act 1981, the Minister of Health hereby renews the provisional consent to the sale, supply or use in New Zealand of the medicine set out in the Schedule hereto:

Note: This consent is given subject to the following conditions:

The vaccine may only be marketed or distributed in accordance with the directives contained in the current version of the New Zealand Influenza Pandemic Action Plan.

Note: This renewed consent is valid for two years from 25 July 2015.

Declaration of Transfer of Ashburton District From South Canterbury Health District to Canterbury Health District

Pursuant to section 19 of the Health Act 1956, I, Teresa Wall, Acting Director-General of Health, declare that Ashburton District shall be transferred from South Canterbury Health District to Canterbury Health District, reflecting the District Health Board geographical areas in Schedule 1 of the New Zealand Public Health and Disability Act 2000.

The boundaries of Canterbury Health District shall be deemed to include Ashburton District.

Consequential to the transfer of Ashburton District to the Canterbury Health District, the South Canterbury Health District shall no longer include Ashburton District.

This declaration will come into force on 1 July 2015.

Declaration of the Chatham Islands Health District

Pursuant to section 19 of the Health Act 1956, I, Teresa Wall, Acting Director-General of Health, declare that the health district of the Chatham Islands shall be established.

The boundaries of the Chatham Islands Health District shall be the district formerly referred to as Chatham Islands County, and is the district known as Chatham Islands Territory under the Chatham Islands Council Act 1995. This comprises the islands known as the Chatham Islands and the area of territorial sea adjoining its lands.

Consequential to the establishment of the Chatham Islands Health District, Chatham Islands County will no longer form part of Hawke’s Bay Health District.

This declaration will come into force on 1 July 2015.
