Notice
- Title—(1) This notice is the Advice Notice to General Practitioners Concerning Patient Benefits and Other Subsidies (Amendment No. 13).
(2) In this notice, the Advice Notice to General Practitioners Concerning Patient Benefits and Other Subsidies[^1] is called the "principal notice".

Commencement—This notice comes into force on 1 July 2012.

Amendment—Schedule 3 of Part B of the principal notice is amended as follows:
(a) Clause 4.6 is amended by omitting "$19.12" and substituting "$19.40" in both places where it occurs.
(b) Clause 5.5 is amended by omitting "$19.12" and substituting "$19.40".
(c) Clause 5.7 is amended by omitting "$19.12" and substituting "$19.40".

Statement of reasons for departure from terms and conditions of the principal notice for purposes of section 89(5) of the New Zealand Public Health and Disability Act 2000—The terms and conditions in this notice depart from the terms and conditions in the principal notice for the following reason:
• to increase the subsidised fee for immunisations by 1.49%;

Civil Union Celebrants for 2012 Notice No. 11
Pursuant to the provisions of section 28 of the Civil Union Act 2004, the following person has been removed from the list of civil union celebrants under section 29 of the Act:
Mullins, Graham Harold, 175 Steel Road, Ararimu, Drury, Franklin.
This removal comes into force on 12 July 2012.
