The revocation of this appointment takes effect on the date this notice is published in the New Zealand Gazette.
Under section 140 of the Intellectual Disability (Compulsory Care and Rehabilitation) Act 2003, I revoke the appointment of the following person as a compulsory care co-ordinator for the named geographical and operational area listed below.
