Reappointment to Canterbury District Health Board (DHB)

Pursuant to section 29 (1) (b) of, and clause 2 of Schedule 3 to, the New Zealand Public Health and Disability Act 2000, and notice to the member under section 28 of the Crown Entities Act 2004, I reappoint

as a member of the board of Canterbury DHB;

and, pursuant to clause 10 (3) of Schedule 3 to the New Zealand Public Health and Disability Act 2000, I also reappoint
