1. Title and commencement—(1) This notice may be cited as the Notice of Fees Prescribed by the Medical Laboratory Science Board.

(2) This notice comes into force on 16 October 2006.

2. Tax—These fees are inclusive of goods and services tax (GST).

A. REGISTRATION AND CHANGE TO SCOPE OF PRACTICE FEES

Pursuant to section 130 (1) (a) of the Act, the fee for application to be registered in the scope of practice of
medical laboratory scientist under section 17 of the Act for a person holding a tertiary educational
qualification gained in New Zealand 230.00
Pursuant to section 130 (1) (a) of the Act, the fee for application to be registered in the scope of practice
of medical laboratory scientist under section 17 of the Act for a person holding educational qualifications
gained elsewhere than in New Zealand 480.00
Pursuant to section 130 (1) (b) of the Act, the fee for application to be registered with a change to an existing
scope of practice under section 17 of the Act 230.00
Pursuant to section 130 (1) (a) of the Act, the fee for application to be registered in the scope of practice of
medical laboratory technician 120.00
Pursuant to section 130 (1) (a) of the Act, the fee for application to be registered in the scope of practice of
medical laboratory technician under section 17 of the Act for a person holding educational qualifications
gained elsewhere than in New Zealand 350.00

B. ACCREDITATION FEES

Pursuant to section 130 (1) (i) of the Act, the fee for carrying out an accreditation review of an undergraduate
degree programme 12,000.00

C. SUPPLY OF DOCUMENT FEES

Pursuant to section 130 (1) (d) of the Act, the fee for the issue of a certificate of registration 45.00
Pursuant to section 130 (1) (e) of the Act, the fee for the issue of a certificate of registered information 45.00
Pursuant to section 130 (1) (g) of the Act, the fee for supplying a medical laboratory scientist or medical
laboratory technician any document for the purposes of seeking registration overseas 100.00
Pursuant to section 130 (1) (d) of the Act, the fee for the issue of a replacement annual practising certificate
declared lost 50.00
