  1. The total remuneration to be reported is:

(a) The amount paid in salary, wages, and other payments, whether in the form of bonuses or otherwise, in return for services. This includes any allowances, extra pays or gratuities, recruitment or retention payments, payments on account of a personal liability or expense incurred by the principal and accommodation or housing subsidy;

(b) The amount paid in connection with the termination of employment by resignation, retirement, redundancy or dismissal, including any compensation or ex gratia payment in settlement of an employment relationship problem or personal grievance; and

(c) The value of any benefits and other emoluments (whether in money or not) in return for services. This includes private use of a motor vehicle provided by the board, subsidised transport, loans, employer contributions to a personal superannuation plan and payments of premiums to income replacement, life or health insurance plans.

  1. The amount specified in sections 1 (a), 1 (b) and 1 (c) respectively shall be separately reported in $10,000.00 bands for each section, except that if the amount is less than $10,000.00 then that amount shall be reported in $1,000.00 bands under the relevant section.

  2. The total remuneration of each principal or acting principal employed during the financial year shall be reported separately.

  3. The reporting requirements set out in this notice shall apply to the annual report for the 2004 financial year, and each subsequent financial year.

Human Assisted Reproductive Technology Act 2004

Approval of Auditing Agency

Pursuant to section 81 (4) of the Human Assisted Reproductive Technology Act 2004, the Director-General of Health approves the Reproductive Technology Accreditation Committee (“the auditing agency”) to audit the provision or likely future provision of the following kinds of health care services:

  • Fertility services

This approval is not subject to any conditions.

Under section 81 (6) of the Human Assisted Reproductive Technology Act 2004, this approval will expire immediately before the commencement of the first notice under section 13 of the Health and Disability Services (Safety) Act that approves standards for fertility services.
