Notice

  1. Title and commencement—This notice may be cited as the Physiotherapy (Fees) Notice September 2004 and shall come into force on 18 September 2004.

  2. Fees—The Physiotherapy Board sets the fees and levies, payable to the board, specified in the Schedule to this notice.

  3. Tax—The fees are inclusive of goods and services tax.

Application for registration within the scope of practice of physiotherapist:
Under section 12 (2) (b) of the Act (New Zealand graduate) 180.00
Under section 12 (2) (c) of the Act (overseas graduate) 600.00
Under Trans-Tasman Mutual Recognition Act 1997 180.00
Application for registration within the scope of practice of physiotherapist – postgraduate student or physiotherapist – visiting educator/presenter:
Under section 12 (2) (c) of the Act – postgraduate student 180.00
Under section 12 (2) (e) of the Act – visiting educator/presenter 180.00
The issue of a practising certificate under section 26 of the Act 333.00
The issue of an interim practising certificate under section 31 of the Act 333.00
For the review of a decision to decline to issue a practising certificate 135.00
For amending the Register (excluding name and address changes) 45.00
For the maintenance of an entry on the Register for 12 months whilst not holding a practising certificate of any kind (non-practising) 45.00
For the restoration of entries in the Register under section 145 of the Act 135.00
On application for change of scope of practice 45.00
For the review of a decision to decline change of scope of practice 135.00
For inspection of the Register and documents 45.00
For supplying a copy of any register entry 45.00
For supplying any document for the purpose of enabling a physiotherapist to seek registration outside New Zealand 45.00
The issue of any other certificates under the Act 45.00
