Title and commencement—This notice may be cited as the Physiotherapy (Fees) Notice September 2004 and shall come into force on 18 September 2004.

Fees—The Physiotherapy Board sets the fees and levies, payable to the board, specified in the Schedule to this notice.

Tax—The fees are inclusive of goods and services tax.
