Removal of Appointed Members Subsequently Elected to Boards

As they were elected to office at the DHB elections in October 2004, pursuant to clause 8 of Schedule 3 to the NZPHD Act, I hereby remove from office as appointed members of the board of the DHBs listed in Column 2 of the table below, the persons named in Column 1 of that table whose name appears alongside the name of the DHB. These removals from office as appointed members take effect from the end of 5 December 2004.

Removal of Appointed Members

Pursuant to clause 8 of Schedule 3 to the NZPHD Act, I hereby remove from office as appointed members of the board of the DHBs listed in Column 2 of the table below, the persons named in Column 1 of that table whose name appears alongside the name of the DHB. These removals from office take effect from the end of 5 December 2004.

These members are being removed from office so that newly appointed and elected members can take office together on 6 December 2004. Their early removal is for administrative reasons only and does not reflect on their performance as board members.
