Auckland DHB

Pursuant to section 29 of the New Zealand Public Health and Disability Act 2000 (“the Act”), I appoint

as a member of the board of Auckland DHB.

This appointment takes effect on the date this notice is published in the New Zealand Gazette and will end on 9 January 2005.

Hutt DHB

Pursuant to clause 10 of Schedule 3 to the Act, I appoint

as chairperson of the board of the New Zealand Blood Service.

Warren Crawford Young’s appointment takes effect on 1 July 2003 and ends on 30 June 2005.
