You must put a tick opposite the name of every candidate you wish to vote for. You may vote for candidates associated with any of the schools named below. You do not have to restrict your choice to candidates associated with the school which your child attends.

If you deliver this paper or have it delivered your vote will not be counted unless the paper is delivered to the Returning Officer before noon on the day of election.
