You must put a tick opposite the name of every candidate you wish to vote for. You may vote for candidates associated with any of the schools named below. You do not have to restrict your choice to candidates associated with the school which your child attends.

You must not put a tick against more than __ names.

You may put a tick against fewer names.

Your vote will not be counted unless this paper is delivered to the Returning Officer in the envelope included with the voting paper. If you post this paper your vote will not be counted unless it is posted to: __ (address) and postmarked before noon on the day of the election and received by the Returning Officer not later than 5 days after the day of the election. If you deliver this paper or have it delivered your vote will not be counted unless the paper is delivered to the Returning Officer before noon on the day of election.
