New Zealand Public Health and Disability Act 2000

Appointments of Members and Chairpersons of District Health Boards

Under section 29 of the New Zealand Public Health and Disability Act 2000 (“the NZPHD Act”), I appoint, as board members of the District Health Boards listed in Column 2 of the table below, the persons named in Column 1 of that table whose name appears alongside the name of the District Health Board.

Under clause 10 of Schedule 3 of the NZPHD Act, I appoint those members as chairpersons of the District Health Board to which they have been appointed.

The appointment of those persons as members and chairpersons is for the period of time specified in the relevant row of Column 3 of that table that appears alongside the name of the District Health Board to which they have been appointed.

Each appointment begins on 10 December 2001.
