Notice

  1. This notice shall be cited as the New Life Christian Academy, Gisborne (Attendance Dues) Notice 2000.
  2. The proprietor of the above-named school may enter into an agreement with parents or other persons accepting responsibility for the education of a child at the above-named school requiring them to pay attendance dues.
  3. The attendance dues payable in respect of any pupil shall be up to a maximum of $400.00 (exclusive of G.S.T.) per annum, per pupil.
  4. Attendance dues received by the proprietor shall be used for the purposes of paying for such improvements to, or for such capital works associated with, the buildings and associated facilities of the above-mentioned integrated school as may be required or approved by the Minister of Education, pursuant to section 40 (2) of the Private Schools Conditional Integration Act 1975, or for meeting debts, mortgages, liens or other charges associated with any of the land and buildings that constitute the integrated school.

Supplementary Integration Agreement
Pursuant to section 10 of the Private Schools Conditional Integration Act 1975, notice is given that a supplementary integration agreement has been signed between the Minister of Education on behalf of Her Majesty the Queen, acting through the Senior Manager, National Operations, Ministry of Education, pursuant to delegated authority, and the proprietor of the following school:

Nelson Christian Academy, Nelson.

The said supplementary integration agreement was executed on the 13th day of July 2000.

A copy of the supplementary integration agreement is available for inspection without charge by any member of the public at the district office of the Ministry of Education, 123 Victoria Street, Christchurch.

Medicines Act 1981

Renewal of Provisional Consent to the Distribution of a New Medicine
Pursuant to section 23 of the Medicines Act 1981, the Minister of Health hereby provisionally consents to the sale, supply or use in New Zealand of the new medicine set out in the schedule hereto:

Dosage Form: Film coated tablet
New Zealand Sponsor: Roche Products (New Zealand) Limited
Manufacturer(s): F Hoffmann-La Roche AG, Basle, Switzerland

Note: This consent is valid for 2 years from 17 December 2000.

This consent is given subject to the following conditions:
(i) The medicine is prescribed only by medical practitioners vocationally registered under the Medical Practitioners Act 1995 in the branch of internal medicine;
(ii) Patients receiving the medicine undergo baseline liver function tests, followed by fortnightly tests for the first 3 months of treatment and 4-weekly tests thereafter;
(iii) Patients are prescribed the medicine only after informed consent has been obtained.
