Casino Control Act 1990
Casino Control Authority Levy
Pursuant to section 76(4) of the Casino Control Act 1990, the Casino Control Authority specifies the amount of the levies to be paid under section 76 of the Act for the financial year 1 July 1994 to 30 June 1995, being the amount approved by the Minister of Internal Affairs and the Minister of Finance under section 76 (3), to be $2,532,000 (G.S.T inclusive), as follows:
North Island—Initial Casino Premises Licence:
$1,174,500 (G.S.T. inclusive).
South Island—Initial Casino Premises Licence:
$1,357,500 (G.S.T. inclusive).

Privacy Act Code of Practice Notice 1994/3
Pursuant to section 49 of the Privacy Act 1993, I give notice as follows:

This notice may be cited as the Privacy Act Code of Practice Notice 1994/3.

The Health Information Privacy Code 1994:
(a) was issued on 28 June 1994; and
(b) will come into force on 30 July 1994. -
Copies of the code are available for inspection free of charge at the Offices of the Privacy Commissioner, 20 Waterloo Quadrant, Auckland and 44–52 The Terrace, Wellington.

Copies of the code are available for purchase from the Offices of the Privacy Commissioner and from Bennetts Bookshops Limited.

Life Insurance Act 1908
Proposed Release of Deposit
Guardian Assurance PLC - New Zealand Branch (herein called “the company”), has given notice to the Public Trustee that it has ceased to carry on the business of life insurance in New Zealand and that pursuant to section 12 of such Act, proposes to withdraw the deposit which has been made by it with the Public Trustee.
The Public Trustee therefore gives notice pursuant to section 12 of such Act that being satisfied that all liabilities of the company in New Zealand in respect of such insurance business have been provided for, he proposes to release to the company on or after 31 July 1994, the amount deposited with him by the company.
Any objections to the release of the amount deposited should be lodged with the Public Trustee at the Public Trust Office, 117–125 Lambton Quay, Wellington 1, on or before 20 July 1994.

Transit New Zealand
Transit New Zealand Act 1989
Declaring State Highway to be a Limited Access Road (State Highway No. 71): Christchurch
- It is notified that Transit New Zealand, by resolution dated 1 June 1994 and pursuant to section 88 (1) of the Transit New Zealand Act 1989, hereby declares that part of State Highway No. 71 in Christchurch on both sides of the highway commencing at a point located on the central median nose (Kaiapoi side) of the Christchurch Northern Motorway overbridge (RP 0/0.00) and extending to a point located 173 metres (RP 0/6.40) southeast of the projected centreline of Railway Road on the southwestern side and 220 metres (RP 0/6.36) southeast of the projected centreline of Railway Road on the northeastern side of the highway as more particularly shown on Sheet 1 of Plan TNZ CH/53 and accompanying schedule held in the office of the Regional Manager, Transit New Zealand, Christchurch and there available for public inspection, to be a limited access road.

Declaring State Highway to be a Limited Access Road (State Highway No. 7A): Hanmer Springs
It is notified that Transit New Zealand, by resolution dated 1 June 1994 and pursuant to section 88 (1) of the Transit New Zealand Act 1989, hereby declares that part of State Highway No. 7A Hanmer Springs Road on both sides of State Highway 7A commencing at the intersection with State Highway 7 (RP 0/0.00) and extending to the intersection with Bath Street (RP 0/8.82), then on the western side of State Highway 7A from the intersection with Bath Street to a point located 115 metres north of Bath Street (RP 0/8.93) as more particularly shown on Sheet 1 of Plan TNZ CH/54 and accompanying schedule held in the office of the Regional Manager, Transit New Zealand, Christchurch and there available for public inspection, to be a limited access road.
