HOW TO USE THIS FORM

The bidding panels may be used for different tender numbers or any number of bidding panels may be used for the same tender number.

Tenderers may bid for any number of the licence units available under any tender number (unless otherwise stated in the Gazette).

Licence units may be bid for at various premium levels per unit including zero bids. Zero bids should be entered as 0.

Licence units are $2,000 c.i.f. each unless otherwise stated in the Gazette.

One licence will be issued for all successful bids under each tender number (regardless of how many panels are used for that tender number). One invoice will be issued for the total premiums payable under each tender number.
CHECK LIST

Type in the tender number(s) for the goods required as listed in the Call for Tenders.

Type in the number of licence units sought in each tender number.

Type in the amount bid for each licence unit sought. Enter whole dollars only. Enter 0 for zero bids.
IF ON REGISTER AND INFORMATION LABEL ATTACHED:
- Sign form and submit bids.
- Photocopy additional forms if required.
- Sign each copy. Ensure details are clear.
IF ON REGISTER BUT INFORMATION LABEL NOT ATTACHED:
- Complete form — type Legal Name, Address, Telephone No. Registration No. and Importer Code.
- Sign form.
IF NOT ON REGISTER:
- Complete form as above. Leave Registration No. blank.
N.B. REGISTRATION NUMBER: this is your tendering Registration Number supplied to you by the Registrar of Tenderers. This is a number of up to five digits.
IMPORTER’S CODE: this is a six digit figure followed by an alphabetic letter. It is obtained from the Customs Department. It is essential for us to be able to put this number on your licence before we can issue it. The number is also used as a cross check on your identity in the computer programme. This number once supplied to the Registrar also appears on your registration label. If it is not on the label, please obtain and type into the box provided.
