NOTICE OF CHANGE OF SURNAME

KNOW all men by these presents (which are intended to be enrolled in the Registry of the Supreme Court at Palmerston North) that I, JOYCE AGNES LOWRY ALLEN, of Levin, Spinster and Civil Servant, at one time long past called JOYCE AGNES LOWRY SIEVERT, do hereby absolutely renounce and abandon the use of my former surname of SIEVERT and in lieu thereof do assume and adopt the surname of ALLEN, so that I may henceforth be called and known not by my former surname of SIEVERT but by my assumed surname of ALLEN.

And for the purpose of evidencing such change I hereby declare that I shall at all times henceforth in all actions and proceedings, dealings, and transactions, and upon all occasions use and subscribe the surname of ALLEN in lieu of my former surname of SIEVERT.

And I therefore hereby expressly authorize and request all persons at all times to designate and address me by my said surname of ALLEN accordingly.

In witness whereof I have hereto subscribed my adopted and substituted name of JOYCE AGNES LOWRY ALLEN, this 5th day of July, 1945.

PURSUANT to section 222 of the Companies Act, 1933, notice is hereby given that by entry in the minute-book in accordance with section 300 of the Companies Act, 1933, made on the 11th day of July, 1945, the following special resolution was passed :—

“ That the company be wound up voluntarily, and that CLAUDE WHITNEY EVANS, of Christchurch, Public Accountant, be and is hereby appointed liquidator of the company.”
