(a) Each block was examined by a competent person after the application of the test load, and was found to have withstood the load without deformation, and to be free from visible cracks, flaws, or other defects.

(b) The sheaves and pins of each block were removed for the examination covered by paragraph (a).

(d) Name and address of firm or person making the test and examination (Columns 4 and 5 para. (a)):

I hereby certify on behalf of the persons named in paragraph (c) above that the above particulars are correct.
