REPORT OF RESULT OF MEETING OF CREDITORS OR CONTRIBUTORIES.

(Title.)

I, A.B., the Official Assignee at [or, as the case may be], chairman of a meeting of the creditors [or contributors] of the above-named company, summoned by advertisement in the newspaper of the , 19 , and in the Gazette of the 19 , and by notice dated , 19 , and held on the day of , 19 at , do hereby report to the Court the result of such meeting as follows:-

1.
