A bank receipt must accompany this application showing that there has been paid
in respect of it into the Public Account to the credit of the Consolidated Fund a fee
of £3 if the vehicle is to be used in a licensed passenger-service and a fee of £2 if it is not
to be so used.
(NOTES.—This form is to be used only for renewals. If not a renewal use Form B.
[Attention is directed to the fact that if any information supplied on this form is
false or misleading it renders the applicant liable to a fine of £10.]
