For the purpose of evidencing such my determination I do hereby declare that I shall at all times in all records, deeds, documents, and other writings, and in all actions, suits, and proceedings, as well as in public and private dealings and transactions, and on all occasions whatsoever use and subscribe the said name of “Henry John D’Ath” as my name, and so that I may be called, known, or distinguished by the name of “Henry John D’Ath.”
