NOTICE TO REMEDY DEFECTS IN THE SANITARY CONDITIONS IN OR ABOUT A MANUFACTURING DAIRY.

TAKE notice that the [Kind of manufacturing dairy] registered under number __, whereof you are the registered owner, is in an unsatisfactory state by reason of the following defects in the sanitary condition of the __ and its appliances: that is to say, [Set out the defects]; and you are hereby required to remedy these defects within __ days after the service upon you of this notice.
