PERSON NAMED, TO TAKE POSSESSION OF CHILD.

Dominion of New Zealand.
WHEREAS on last past complaint was made before me, the under-
signed [

has the custody of a child named [Full name of child], and that the said
child is a *neglected [or indigent, or delinquent] child [or is not under proper

forthwith removed from its surroundings].

deliver it into the custody of any person for the time being in charge of
