I, , Comptroller of Customs for the Dominion of New Zealand, do hereby certify that the importation of the under-mentioned drugs by is approved by the Government of the said Dominion :- Name, Nature, and Description of Opium, Morphine, Heroin, Cocaine, Codeine, Ecgonine, or of Preparation thereof. Quantities to be imported. Name and Address of Person to whom the Order for the Drugs is to be sent. Name and Address of Person from whom the Drugs are to be obtained (if available). I further certify that the said drugs are required for legitimate purposes, and that any of the said drugs to which Chapter III of the International Opium Convention, 1912, applies are required solely for medicinal or scientific purposes. Keep a separate page of the book for entering the receipt and disposal of each form of opium--e.g., raw, solid extract, tincture, liquid extract, &c.,--and a separate page for morphine, for heroin, for cocaine, for codeine, and for ecgonine, and their respective salts. (b.) Any opium used in the manufacture of a preparation containing opium which cannot be made suitable for smoking need not be entered in the columns headed "Sold, exported, or otherwise disposed of," provided the necessary particulars are entered in the columns headed "Used in manufacture." (c.) Where an importer uses, either in a manufacturing warehouse licensed under the Customs Acts or elsewhere, any dangerous drug in the manufacture of another dangerous drug (other than a preparation containing opium which cannot be made suitable for smoking), such last-mentioned dangerous drug shall be deemed to be "received" for the purposes of keeping the Dangerous Drugs Book, and shall be entered therein accordingly, (See Regulation No. 7 (2).)
