Sec. 22.] [Form S. 21.
Under the Shops and Offices Act, 1921–22.
HOLIDAY BOOK FOR SHOPS (OTHER THAN HOTELS AND RESTAURANTS).

Name of Assistant. Nature of Employment. Working-day fixed and observed as the Half-holiday.

Section 22 of the Act requires that this book shall be open at all times to inspection by an Inspector or by any assistant employed by the employer.

Sec. 24.] [Form S. 22.
Under the Shops and Offices Act, 1921–22.
APPLICATION FOR PERMIT TO KEEP SHOP OPEN ON STATUTORY CLOSING-DAY.

To the Inspector of Factories,

I, __, being the occupier of a shop situated at __, in which the principal business carried on is the sale by auction to consumers of fish, fruit, vegetables, meat, or other perishable goods of a like nature, namely, __, hereby apply for a permit to keep the said shop open and employ my assistants on the __ day of __, 192____, for the sale of such goods only on the following grounds: [State grounds showing that it is necessary that the shop be kept open in order to prevent such goods from deteriorating].

Under the Shops and Offices Act, 1921–22.
PERMIT TO KEEP SHOP OPEN ON STATUTORY CLOSING-DAY.

I, __, an Inspector of Factories, being satisfied that __ is the occupier of a shop situated at __, in which the principal business carried on is the sale by auction to consumers of fish, fruit, vegetables, meat, or other perishable goods of a like nature, namely, __, and that it is necessary that the said shop be kept open on the __ day of __ in order to prevent such goods from deteriorating, hereby permit the said occupier to keep his shop open accordingly and to employ his assistants for the sale of such goods only.

NOTE.—Every assistant employed on the statutory closing-day in accordance with section 24 must be paid for all time so worked at not less than one-half as much again as the ordinary rate, and shall be given equivalent time off within seven days thereafter.

PERMIT TO OBSERVE ANOTHER DAY AS STATUTORY CLOSING-DAY.

I, __, an Inspector of Factories, being satisfied that __ is the occupier of a shop situated at a holiday resort, namely, __, hereby permit the said occupier to observe __ as the statutory closing-day for the said shop in lieu of __.

NOTE.—This permit is issued on the express condition that it may be cancelled at any time by an Inspector of Factories.

NOTICE OF APPEAL.
(From Requisition of Inspector of Factories.)

In the matter of the Shops and Offices Act, 1921–22, and of a requisition by __, of __, an Inspector under the said Act, dated the __ day of __, 192____.

NOTICE is hereby given that I [or we], __, the occupier upon whom the said requisition has been served, consider such requisition to be unreasonable, and desire to appeal therefrom. The grounds of the appeal are set forth in the attached memorandum, marked “A.” A true copy of the said requisition is also attached hereto, marked “B.”

APPLICATION FOR EXEMPTION OF CHILD OF OCCUPIER.

To the Inspector of Factories,

I, __, the occupier and owner of the business carried on in the shop situated at __, hereby apply for the exemption from all the provisions of this Act relating to shop assistants of __ son [daughter], there being no other assistant employed in the said shop.

Sec. 70.] [Form S. 29.
Under the Shops and Offices Act, 1921–22.
CERTIFICATE OF EXEMPTION OF CHILD OF OCCUPIER.

I, __, an Inspector of Factories, hereby exempt [Name of son or daughter of occupier, as case may be] from all the provisions of the Shops and Offices Act, 1921–22, relating to shop assistants, if and so long as the said __ is engaged in the shop of __, his [her] father [mother], and if and so long as no assistants other than the said child are employed in the said shop.

NOTES.—This exemption is granted on the express condition that it may be cancelled at any time by an Inspector of Factories.

Nothing in section 70 shall entitle any shop to remain open after the hours fixed in accordance with this Act.
