  1. Every Deputy Registrar shall despatch to the Registrar, not later than the third day in every month, a return, in the form numbered 24 in the Schedule hereto, of all pension-certificates issued by him during the preceding month, together with a list of pensioners whose pension-year expired at the end of the month but one preceding, and whose application for a new certificate was either not made or not granted up to the date of the return.

  2. The Registrar shall furnish to the Postmaster-General monthly a schedule containing the names of all pensioners, the numbers of their respective pension-certificates, the respective money-order offices at which the next instalment of each pension is payable, and the amount of each instalment.

  3. On the death of any person within the colony of the age of sixty-five years and upwards, it shall be the duty of the Registrar of Deaths to whom the death is reported to inquire, and it shall be the duty of the person reporting the death to inform the said Registrar, whether the deceased person was a pensioner under the said Act; and the said Registrar shall send to the Registrar of Old-age Pensions notice of the death, in the form numbered 25 in the Schedule hereto.

  4. Every statutory declaration made pursuant to these regulations shall be exempt from stamp duty.

  5. Letters addressed to the Registrar or any Deputy Registrar by his official name, and also letters sent by the Registrar or any Deputy Registrar on official business shall be exempt from postage.

  6. No Court fees shall be payable in connection with any proceedings before a Magistrate or Justices under these regulations.

  7. The search-fees prescribed by sections 16 and 57 of the principal Act shall be prepaid in stamps in manner provided by the regulations for the time being in force under “The Stamp Act, 1882.”

PENSION-CLAIM.

I, [Full name], hereby make claim for a pension under the aforesaid Act, and in support thereof do hereby solemnly and sincerely declare as follows:—

  1. My full name is [Full name].

  2. I reside at [Place of residence, and postal address], within the [Name of district] Old-age Pension District.

  3. My occupation is [If of no present occupation, say so, and state the last].

  4. I am [State whether single, married, widow, or widower. If married, give full name of husband or wife, and if a widow or married woman, give maiden name. If married, state whether husband and wife are living together, or whether they are living apart pursuant to decree, order, or deed of separation, and also state whether husband or wife is in receipt of an old-age pension].

  5. I was born at [Place], in [Country], on the day of , 18 , and my age last birthday was years.

  6. I first arrived in New Zealand on or about the day of , 18 , by the [Name of ship, and, if possible, name of captain]. [If the claimant was born in New Zealand, this paragraph may be struck out, or left in blank.]

  7. During the period of twenty-five years immediately preceding the date of this my pension-claim I have been absent from the colony on [Set out number] occasions only, particulars whereof are as follows: [Set out in respect of each occasion the date and port of departure, the ship and destination, and also the date and port of return and the name of the ship]. [If the claimant has not been absent from the colony at all during the twenty-five years he must alter this paragraph to express the fact.]

  8. I am possessed of [Set out particulars of property (if any), whether land or otherwise].

  9. To the best of my knowledge and belief I have fulfilled all the requirements and am exempt from all the disqualifications under the aforesaid Act up to the date of this my pension-claim, and am entitled to a pension.

  10. The contents of this my pension-claim are true and correct in every material point.

And I make this solemn declaration conscientiously believing the same to be true, and by virtue of the provisions of an Act of the General Assembly of New Zealand intituled “The Justices of the Peace Act, 1882.”

INSTRUCTIONS TO CLAIMANTS.

NOTE.—The special attention of the claimant is drawn to the following points:—

By section 8 of “The Old-age Pensions Act, 1898,” as amended by section 2 of the Amendment Act, 1900, it is enacted that no person shall be entitled to a pension unless he fulfils the following conditions, that is to say,—

By section 64 it is enacted that pensions shall not be granted to—

By section 13 it is enacted that the following general rules shall apply in the computation of income for all the purposes of the Act:—

By section 10 it is enacted that the net capital value of accumulated property shall be computed as follows:—

By section 12 it is enacted that, for the purpose of ascertaining whether the claimant is entitled to a pension, and also of fixing the rate of the first year’s pension, his income for the next-preceding income-year (meaning thereby the year ending one month before the date on which the pension-claim is finally admitted) shall be deemed to be his yearly income.

On the investigation of the claim before the Stipendiary Magistrate the claimant must be prepared with evidence in corroboration of his own that he has fulfilled all the foregoing conditions up to the date on which the claim is established; and in particular he must produce a copy of the Government valuation of any land owned by him, and evidence of any mortgage thereon; and in case he has money in the savings-bank he must produce the bank pass-book.

He must in like manner be prepared with evidence showing the amount and full particulars of his income for the year ending one month before the claim is established, and of the accumulated property owned by him at the ending of such year.
