EXTRACT from the OFFICIAL LOG BOOK of PARTICULARS OF BIRTHS which have occurred on board

NOTE.–In pursuance of the power invested in the Board of Trade under the provisions of section 37 of "The Births and Deaths
Registration Act, 1874," the Board of Trade directs that the return of all births and deaths which occur on board any
British merchant vessel shall be recorded in the Official Log Book in the form provided therein. Upon the arrival of
the ship at any port or place out of the United Kingdom, a copy of the record must be delivered to the Principal
Consular Officer, the Shipping Master, or Collector of Customs, for the transmission of a return of the entry to the
Registrar-General of Shipping and Seamen. On arrival at the port of destination in the United Kingdom, the Official
Log Book must be deposited with the Superintendent of Mercantile Marine, to be forwarded in due course to the
Registrar-General of Shipping and Seamen.

occurred on board the Ship

NOTE.–In pursuance of the power invested in the Board of Trade under the provisions of section 37 of "The Births and Deaths
